When making an appointment, please let us know if you are suffering from a work related injury or motor vehicle accident issue or injury, as additional authorization will be needed from your Adjuster or Nurse Case Manager to schedule your appointment. If this information is not received by the time the appointment is made, there is a possibility you will need to be rescheduled when you arrive for your appointment.
